We are currently seeking experienced LPNs & RN's to fill job openings. The ideal candidate will possess a current LPN license with excellent clinical skills

Requirements of the LPN:
- Photo for ID Badge (Take a Photo from your Camera)
- (2) Pieces of ID (Driver’s License/Passport/Resident Card/work authorization card)
- Social Security Card
- NYS LPN License
- NYS Registration
- CPR Certification
- Fire Safety -- (www.ceufast.com)
- Infection Control -- (www.Ceufast.com)
- Malpractice Insurance
- Reference (1- Previous Employer and 1-Personal)
- Direct Deposit form or Void Check (Must be signed)
- COVID Vaccination
- Flue Vaccine (Not required)
- Complete Physical Exam -- (MUST BE WITHIN 6 MONTHS FROM THE DATE OF APPLICATION)
MMR (Measles, Mumps, Rubella or Rubeola) Lab Reports
Chest X-Ray Lab Report (if positive PPD)
