Your Responsibilities Will Include:
- Implementing care plans under the supervision of a licensed Physical Therapist
- Documenting patient progress, treatment notes, and discharge summaries accurately and timely
- Assisting patients in performing therapeutic exercises and daily activities
- Promoting a safe, positive, and encouraging rehabilitation environment
Qualifications:
- Valid New York State Physical Therapist License
- Experience in Skilled Nursing Facilities (SNF) and/or Acute Care is preferred
- Familiarity with Rehab Optima or other EMR systems is a plus
- Must be fully vaccinated against COVID-19
