Duties
- Conduct thorough occupational therapy assessments to evaluate patients’ functional abilities and needs across diverse populations including pediatrics, adults, and seniors.
- Develop and implement evidence-based treatment plans tailored to each patient’s goals, medical conditions, and living situations.
- Assist patients with Activities of Daily Living (ADLs), such as dressing, grooming, feeding, and mobility to promote independence.
- Support discharge planning by coordinating care transitions and providing education to patients and caregivers on therapy exercises and adaptive strategies.
- Collaborate with physicians, speech therapists, physical therapists, educators, and case managers through clinical interprofessional collaboration to optimize patient care.
- Maintain accurate medical documentation within electronic health records (EHR), ensuring compliance with healthcare regulations and standards.
- Provide specialized interventions for individuals with neurodevelopmental disorders like autism spectrum disorders and support early intervention programs for children with disabilities.
- Educate patients and families on occupational health strategies, safety modifications at home, and adaptive equipment usage.
- Participate in outpatient clinics, inpatient units, skilled nursing facilities, or home health visits based on program needs.
Requirements
- Valid state licensure or certification as an Occupational Therapist with eligibility for home health practice.
- Proven experience working with individuals with disabilities across settings such as outpatient clinics, inpatient units, skilled nursing facilities, or home health environments.
- Strong knowledge of anatomy, physiology, medical terminology, and evidence-based occupational therapy practices.
- Experience working with pediatric populations including early intervention and school-aged children with disabilities or special education needs.
- Demonstrated ability to develop treatment plans focused on functional improvements and client-centered goals.
- Skilled in case management tasks such as discharge planning and coordinating multidisciplinary care teams.
- Proficiency in electronic health records (EHR) management and clinical documentation standards.
- Excellent client communication skills to effectively educate patients and caregivers about therapy strategies and safety measures.
- Ability to work independently while maintaining strong collaboration within interprofessional healthcare teams.
- Prior inpatient experience or skilled nursing facility background is preferred; experience working with neurodevelopmental disorders like autism spectrum disorder is highly valued.

Pay: $48.10 - $65.00 per hour
