Position: Residential Nurse
Duties:
- Develop and/or update medical care plans addressing health concerns of each consumer.
- Complete and annually update life-time medical history for consumers
- Review the consumers’ individual health status, including dietary needs.
- Review medical and medication policies and procedures.
- Train and educate staff persons regarding the needs of the individuals, including infection control.
- Respond to emergencies involving the consumers’ health and stability and intervene when necessary for unstable medical episodes.
- Attend in-service training, staff meetings, and conferences as needed.
- Able to effectively communicate with and maintain conducive relationship with other individual involved in the care of each consumer (physicians, staff, team members, family etc.) with regard to concerns or implementation of care in regard to the welfare of the individuals.
- Effectively and clearly express thoughts and ideals verbally and in writing.
- Maintain interest in positive improvement and development of person with disabilities.
- Able to analyze and interpret and summarize medical records and data. Review all medical visit reports, physical forms, lab work/test results, medical logs and prescriptions on a regular basis.
- Carries on-call phones for medical concerns and emergencies.
- Review all medical information provided regarding consumers.
- Obtain medical information and preparing reports needed for more medically involved individuals.
- Coordinate all discharges from the hospital back to the individual’s home.
- Visit the consumers at their home and/or day program to assess and evaluate medical concerns.
- Assess and chart observations of the patient's condition at each visit
- Complete evaluation tasks, including reviewing medication and vital signs
- Administer Physician-prescribed medication
- Dress or redress wounds and assess the healing progress
- Coordinate with physicians, physical therapists, occupational therapists and other individuals in the patient's care plan
- Recommend tools or devices that may improve the quality of life for the patient
- Monitor changes in behavior and progress.
- Responsible for HRST clinical reviews and update
- Schedule and follow up on all routine and follow-up appointment
- Coordinate monthly medication delivery from the pharmacy. Audit medication for completeness and accuracy
- Perform other duties as requested by superior.
Qualifications:
Registered nurse with a current license in this commonwealth or a practical nurse, currently licensed in this commonwealth who is currently under the direction of a licensed professional nurse or a licensed physician.
Experience working with aging adults and adults with functional impairment(s). Clear criminal history and child abuse check.
