Bear Hill Rehabilitation and Nursing Center is looking for a RN Case Manager to join their team.
Position Summary:
The RN Case Manager is responsible for coordinating and overseeing resident care to ensure the highest quality outcomes, regulatory compliance, and resident satisfaction. The RN Case Manager serves as a liaison between residents, families, physicians, interdisciplinary team members, and outside providers to facilitate comprehensive care planning and effective care transitions.
Essential Duties and Responsibilities:
- Conduct comprehensive resident assessments and participate in the development, implementation, and evaluation of individualized care plans.
- Coordinate care among nursing staff, physicians, therapy departments, social services, dietary, and other interdisciplinary team members.
- Monitor resident progress and clinical outcomes, identifying and addressing changes in condition promptly.
- Review and manage hospital admissions, readmissions, discharges, and transfers to ensure continuity of care.
- Participate in care plan meetings with residents and families, providing education and support regarding care needs and goals.
- Collaborate with physicians and nurse practitioners to ensure timely communication and implementation of orders.
- Assist with utilization review, clinical documentation, and case management processes to support quality care and reimbursement.
- Ensure compliance with federal, state, and facility regulations, policies, and procedures.
- Monitor and track quality measures, identifying opportunities for performance improvement.
- Facilitate referrals to specialty services, community resources, and outside providers as needed.
- Advocate for residents' rights, preferences, and overall well-being.
- Participate in quality assurance and performance improvement (QAPI) activities.
- Maintain accurate and timely clinical documentation within the electronic medical record.
- Assist with infection prevention, risk management, and clinical initiatives as assigned.
- Support discharge planning and coordination of post-discharge services to promote successful transitions.
Qualifications:
- Current Registered Nurse (RN) license in good standing.
- Minimum of 2 years of nursing experience; long-term care or skilled nursing experience preferred.
- Strong knowledge of nursing home regulations, MDS process, and care planning.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work collaboratively with interdisciplinary teams.
- Proficiency with electronic medical records and Microsoft Office applications.
- Case management or care coordination experience preferred.

- Ability to respond quickly in emergency situations.
- Ability to communicate effectively with residents, families, staff, and healthcare providers.
