Duties
- Provide direct support to residents by assisting with activities of daily living (ADLs), including personal care, grooming, and mobility assistance using manual handling and patient transfer methods such as Hoyer lifts.
- Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes in accordance with care plans and HIPAA regulations to ensure safety and compliance.
- Administer medications and assist with medication management following prescribed protocols to support ongoing health needs.
- Prepare nutritious meals and assist with meal planning while ensuring dietary restrictions are followed, supporting overall health and wellness.
- Maintain a clean, safe environment by performing regular cleaning duties, laundry, and ensuring infection control standards are met in line with healthcare regulations.
- Support residents’ participation in social activities and community outings through personal transportation assistance, fostering social inclusion and engagement.
- Document care activities accurately in resident records, communicate effectively with team members about patient status, and participate in care planning meetings to tailor support strategies.
Qualifications
- Prior experience working with individuals that require CLS services, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Knowledge of dementia care, Alzheimer’s care, memory care, autism support, neurodevelopmental disorders, intellectual disabilities support, and developmental disabilities is advantageous.
- Certification or experience in CPR and First Aid is required; additional certifications such as HIPAA training or hospice care experience are a plus.
- Ability to perform heavy lifting safely and utilize patient transfer methods like Hoyer lifts; manual handling skills are essential.
- Strong communication skills for client interaction and team collaboration; experience working with diverse populations including geriatrics is desirable.
- Familiarity with patient monitoring tools such as vital signs measurement and medication administration protocols.
- Demonstrated ability to follow detailed care plans while maintaining confidentiality according to HIPAA standards.
