Responsibilities:
- Schedule appointments and transportation for long term residents and keep track of reoccurring appointments
- Contact resident’s families for consents, visits, and care conferences
- Assist team with assessments and monitor resident’s behaviors
- Help residents shop and reach out to families to bring residents new clothing or take away old clothing
-Assist the Social Services team with DME orders, appiontments, surveys, and other tasks related to patient discharges
-Make post-discharge calls to follow up with patient progress
-Must be able to work weekends
Job Type: Full-time
Pay: $23.00 - $28.00 per hour
Expected hours: 32.0 – 40.0 per week
