Nova Health First is hiring a licensed Physical Therapist on a PRN basis to deliver skilled therapy to patients in their homes across the Indianapolis metro. Choose the visits that fit your schedule — we provide a steady stream of referrals, responsive office support, and straightforward documentation in WellSky.
Physical Therapist – PRN (Home Health) Responsibilities
- Conduct evaluations, re-evaluations, and discharges
- Complete OASIS-E assessments (SOC, ROC, Recert, Discharge) as assigned
- Develop and update individualized plans of care
- Deliver skilled interventions for balance, gait, strength, post-surgical recovery, and chronic condition management
- Document same-day in WellSky and coordinate with the interdisciplinary team (OT, SLP, SN, MSW)
Physical Therapist – PRN (Home Health) Requirements
- Active Indiana PT license in good standing
- Can pass a federal and or expanded national background check
- Current BLS/CPR certification
- Valid driver's license, reliable vehicle, and auto insurance
- Home health experience strongly preferred; OASIS-E and WellSky proficiency a plus
