Position Overview:
As an LVN/LPN at Bradbury Commons, you'll be a vital member of our clinical team. You’ll deliver direct nursing care, monitor resident health, and support daily living activities in accordance with each resident's personalized care plan. This is a hands-on role that requires clinical skill, attention to detail, and a deep respect for the aging population.

Provide direct nursing care to residents in accordance with care plans and physician orders

Administer medications and treatments as prescribed

Monitor residents for changes in condition and report promptly to RN supervisor or physician

Document care and observations accurately in EMR system

Assist with ADLs (Activities of Daily Living) when needed

Collaborate with interdisciplinary team to ensure holistic resident care

Participate in care plan meetings, quality improvement initiatives, and staff training

Uphold infection control, safety, and facility policies at all times
Requirements:
Qualifications:

Current and valid LVN or LPN license in the state of employment

CPR certification (Basic Life Support)

Experience in long-term care or assisted living preferred

Strong clinical and communication skills

Compassionate, patient-centered approach

Ability to work effectively in a team and independently

Flexibility for shifts, including evenings, weekends, and holidays as needed
