Graveyard Shift (11pm-8am) full-time ; (8 hours)
Location: Downtown Los Angeles, CA - 90013 (Skid-Row area)
Salary Range: $50-$52/hr + DOE
Schedule/Shift: Flexibility to work weekends on a rotating basis is required.
This patient-centered space is committed to harm reduction and trauma-informed care. Designed as a welcoming drop-in site for people who use drugs and those experiencing homelessness, it provides a safe environment for hospitality, community, and comprehensive care. Unlike traditional clinics, it operates with flexibility, prioritizing accessibility and support over rigid structures.
Principal Responsibilities:
1. Provide compassionate, low-barrier nursing care in alignment with harm reduction principles.
2. Perform triage, patient assessments, and develop individualized care plans.
3. Conduct nurse-only visits, including wound care (for complex chronic wounds), medication reconciliation/management, phlebotomy (for patients with challenging venous access), and complex care coordination.
4. Administer medications and provide essential nursing services per established protocols and standards.
5. Monitor patients in observation beds, responding promptly to changes in condition and ensuring appropriate interventions.
6. Offer health education and risk reduction counseling, including safe drug use practices and overdose prevention.
7. Administer Narcan and train participants/staff in overdose response and harm reduction strategies.
8. Work collaboratively with social workers, case managers, and harm reduction specialists to address physical health, mental health, and substance use needs.
9. Support participants in navigating the healthcare system, including referrals to specialty care, housing, and social services.
10. Recognize and respond to signs of intoxication, withdrawal, and overdose, ensuring timely intervention and support.
11. Ensure patient care is accessible in non-traditional settings, including drop-in environments and outdoor areas.
12. Provide guidance and support to Medical Assistants.
Requirements:
1. Valid Registered Nurse (RN) license issued by the State of California.
2. Current certification in CPR from the American Heart Association.
3. A minimum of three years' experience in a clinical or healthcare setting, with a strong preference for experience in community health or care for vulnerable populations.
4. Desire to work in a non-traditional clinical setting, including comfort in providing care outside of traditional clinical areas (e.g., outdoor parklets, drop-in spaces, etc.).
5. A demonstrable passion for providing non-judgmental and compassionate care to patients experiencing homelessness and managing complex, often severe, medical and psychiatric conditions.
6. Proficiency in conducting comprehensive health assessments, administering medications, and providing nursing interventions.
7. Exceptional interpersonal skills, cultural competence, and the ability to communicate effectively with diverse populations.
8. Bilingual Ability (Spanish/English) preferred.
