Job Summary
This position requires three or more years experience in a leadership position with individuals who have been diagnosed with an intellectual disability/developmental disability. This position is responsible for providing person centered practices while administering daily care of the individuals who are being supported at The Peninsula Community Care Center, LLC. This position provides person-centered support for activities of daily living (ADLs) The role implements the Individual Support Plan (ISP) by assisting with medication management, doctor's appointments, accurate record-keeping and daily household needs within the program. This position will oversee the Medication Administration Record (MAR) using the Quality Tool to review and ensure medication administration records are accurate and complete. This position will ensure daily support logs are checked and completed, and the MAR document is properly initialed by all staff. This position will communicate directly with the support staff regarding concerns about the individuals, household supplies needed, upcoming visits, medication or ISP changes, and any other important information for the staff. This position will Report to the Executive Director of Programs any staffing concerns/changes, scheduling conflicts, or concerns regarding the individuals care. All other duties will be assigned based on the needs of the individuals and program.
This position will be directly supervised by the Executive Director of Programs.
Duties
- Adhere to all Policies and Procedures of The Peninsula Community Care Center as outlined by DBHDS.
- Provide leadership in delivering person centered supports
- Review and complete daily support logs to ensure they are person centered and documented daily in accordance with DMAS standards and/or regulations (Residential)
- Administer medication and Review Medication Administration Record daily in accordance with five rights and required protocols.
- Use the Medication Quality Tool and Incident Report to document errors.
- Review Medication Monthly intake from pharmacy ensuring accuracy in accordance with doctor's orders and prescription orders.
- Provide daily training, supervision, and assistance to Direct Support Staff for individuals receiving supports in a person-centered manner.
- Responsible for completing Person-Centered Reviews for residents on a quarterly basis.
- Provide feedback and support to Direct Support Professionals.
- Maintain current, accurate and appropriate individual records and other relevant documentation involving individuals and programming.
Pay: $37,000.00 - $41,000.00 per year
