- Health & Wellness Needs - advocacy with healthcare providers, nutrition, meal planning, grocery shopping & meal preparation.
- Use of community resources, including public transportation
- Community integration & social support - including companionship
- Social skills
- Advocacy & mentorship
- Support the individual in leading meaningful lives in the community, including
Tailored Day Services
- Post secondary education – supporting in enrollment in courses, accessing disabled student services or any other accommodation, acclimating to classes, completing coursework and related activities
- Employment – job/internship search, resume review or writing, mock interviews, job application
- Community Integration & Personal Development – safety & survival training, socialization, mobility training, transportation education, social activities
- Maintain proper documentation relating to the individuals
- Communicate & collaborate with Individual’s family, supervisors and co-workers
- Must be Bi-lingual to communicate with individual served
- Performs other duties as required.
Job Types: Full-time, Part-time
Pay: $21.00 - $23.00 per hour
Expected hours: 30 – 40 per week
