Job Summary:
We are seeking a dedicated and detail-oriented Registered Nurse (RN) to join our Home Health team in an in-office and out of office capacity. This role focuses on supporting field clinicians, coordinating patient care, managing documentation, and ensuring compliance with regulatory requirements. Ideal candidates will have strong organizational skills, clinical knowledge, and a passion for improving patient outcomes.
Key Responsibilities:
- Review and process clinical documentation for accuracy, timeliness, and compliance
- Coordinate patient care plans with field staff, physicians, and families
- Assist in scheduling visits and managing clinician caseloads
- Conduct Start of Care (SOC), Resumption of Care (ROC), and Discharge chart reviews
- Provide guidance and clinical oversight to field staff
- Communicate with referral sources and intake coordinators to ensure smooth transitions
- Maintain electronic medical records (EMR) and ensure compliance with agency policies and CMS regulations
- Participate in quality assurance and performance improvement (QAPI) activities
Qualifications:
- Current CA RN (required)
- No experience needed (Training will be held)
- Multilingual (Korean & English)

Pay: $40.00 - $80.00 per hour
