Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and detailed health histories to inform care plans.
- Administer medications, injections, IV infusions, and other treatments with precision and adherence to safety protocols.
- Manage complex patient needs such as pain management, wound care including suturing and sterile techniques, and catheterization procedures.
- Utilize electronic health record (EHR) systems like Epic or Cerner for accurate medical documentation and case management.
- Collaborate with multidisciplinary teams to develop discharge planning strategies and coordinate post-acute care services.
- Conduct diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures when necessary.
- Provide specialized care for individuals with developmental disabilities, dementia (including Alzheimer's and memory care), or those requiring hospice or palliative care.
- Engage in patient education and health coaching to promote wellness and disease prevention across diverse populations.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in Basic Life Support (BLS).
- Demonstrated experience in critical care environments such as ICU or Level I Trauma Centers is highly preferred.
- Proficiency with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks; familiarity with ICD-9/10 coding is advantageous.
- Experience working with specialized populations including pediatrics, geriatrics, neonatal ventilators, or individuals with disabilities.
- Strong knowledge of physiology, anatomy, pharmacology, medical terminology, and diagnostic evaluation techniques.
- Ability to perform complex procedures such as IV insertion, venipuncture, tube feeding management, and airway management confidently.
- Excellent communication skills for triage assessments, patient education, and interdisciplinary collaboration.
- Flexibility to work in various settings including hospital medicine, outpatient clinics, home & community care environments or travel nursing assignments.
