Position Summary
PurposeCare is seeking an experienced and dynamic Administrator / Director of Nursing to lead the clinical and operational success of our Indianapolis/ Fort Wayne Home Health branch. This leadership role is responsible for overseeing daily agency operations, ensuring regulatory compliance, driving census growth, and cultivating a high-performing clinical team focused on delivering exceptional patient outcomes.
The ideal candidate is a collaborative healthcare leader with extensive home health experience, strong operational expertise, and a passion for developing teams while delivering outstanding patient care.
Key ResponsibilitiesClinical Leadership
- Provide leadership and oversight for nursing, therapy, and interdisciplinary clinical teams.
- Promote a culture of clinical excellence, accountability, and patient-centered care.
- Monitor quality metrics, patient outcomes, satisfaction scores, and performance improvement initiatives.
- Mentor, coach, and develop clinical leaders and staff to maximize engagement and performance.
- Ensure timely, accurate clinical documentation and adherence to evidence-based practices.
Operational Leadership
- Direct daily operations to ensure efficient workflows and exceptional patient care delivery.
- Manage staffing, productivity, scheduling, and resource allocation to meet agency goals.
- Partner with Talent Acquisition and Human Resources to recruit, hire, onboard, and retain top clinical talent.
- Develop operational strategies that improve efficiency, financial performance, and patient satisfaction.
- Collaborate with billing and administrative teams to ensure documentation supports timely reimbursement.
Regulatory Compliance
- Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, accreditation standards, and company policies.
- Lead survey readiness efforts and successfully manage state and federal audits.
- Oversee quality assurance, infection prevention, risk management, and patient safety initiatives.
- Maintain regulatory compliance while fostering continuous quality improvement.
Business Development, Operations & Growth
- Partner with Business Development, Referral Partners, and Executive Leadership to support strategic growth initiatives.
- Build and strengthen relationships with physicians, hospitals, skilled nursing facilities, and community partners.
- Drive census growth while maintaining exceptional clinical quality and patient outcomes.
- Support market expansion initiatives and organizational strategic objectives.
Qualifications
- Active Registered Nurse (RN) license in good standing required.
- Minimum of three (3) years of progressive home health leadership experience.
- Previous experience as an Administrator, Clinical Director, Director of Nursing, Branch Director, or similar leadership role preferred.
- Strong working knowledge of Medicare Conditions of Participation (CoPs), state regulations, and accreditation standards.
- Experience with electronic medical record platforms such as Axxess, Homecare Homebase (HCHB), Matrix Care, Alayacare, WellSky, or similar systems preferred.
- Demonstrated success leading interdisciplinary teams and driving operational performance.
- Excellent communication, organizational, leadership, and problem-solving skills.
- Ability to thrive in a fast-paced, patient-focused healthcare environment.

Pay: $110,000.00 - $120,000.00 per year
