Job Title: Medical Assistant
Classification: Full Time/ Non-Exempt
Reports to: Clinical Director
Salary: $17-$20/Hour
Job Summary: The Medical Assistant works collaboratively with medical providers and the care team to support clinical and administrative operations. The role focuses on ensuring patients receive high-quality, comprehensive care by reducing barriers to positive health outcomes. This position is integral to fostering patient engagement, addressing social determinants of health, and connecting patients to essential services.
Key Responsibilities:
Clinical Support:

Prepare patients for medical appointments, including rooming, taking vitals, gathering medical history, and assessing the focus of the visit.

Administer preventive care measures such as vaccinations and facilitate diagnostic tests.

Manage medication refill requests and ensure timely communication with providers.

Assist in patient education, focusing on chronic disease management, preventive care, and health promotion.
Patient Navigation and Advocacy:

Establish trusting relationships with patients, identifying and addressing social determinants of health needs.

Provide care coordination and advocacy for social services such as transportation, food security, and other supportive programs offered in-house or through external agencies.

Assist patients with applications for pharmaceutical assistance programs and other resources.

Maintain a comprehensive and current database of local resources and community services for patient referrals.

Follow up with patients to monitor progress, resolve barriers, and ensure satisfaction with services provided.
Administrative Responsibilities:

Schedule appointments for planned care visits via direct communication or outreach methods.

Complete patient referrals and track their outcomes to ensure effective follow-up.

Document all patient interactions and care coordination activities in Jhe Electronic Medical Record (EMR) system.

Support the call center by answering inquiries, scheduling appointments, and relaying messages to clinical staff.
Team Collaboration:

Work closely with providers and direct care staff to ensure all patient needs are met holistically.

Exhibit excellent communication and interpersonal skills with patients, team members, and community partners.

Contribute to a positive, team-oriented environment to achieve patient care goals.
Requirements:
Qualifications and Experience:

Certification as a Medical Assistant required.

Community Health Worker Certification preferred.

Experience in Federally Qualified Health Centers (FQHCs) or similar healthcare settings highly desirable.

Proficiency with EMR systems and clinical documentation.

Strong organizational skills with attention to detail and ability to manage multiple tasks.

Flexible schedule to include some evenings and weekends as needed.

Ability to lift up to 25 lbs and perform duties in medical office and clinic environments.

Time/Physical Requirements

Some evening and weekend activities may require flexing schedule
