Brief DescriptionThe position of Client Care Coordinator plays a crucial role in coordinating and overseeing the delivery of home care services across non-medical and Medicaid waiver programs. This position requires an office-based professional with strong organizational skills, knowledge of state Medicaid guidelines, and fluent bilingual communication (English/Spanish). A primary focus of this role involves collaborating directly with Case Management Agencies (CMAs) and navigating specialized programs, specifically the In-Home Support Services (IHSS) program and the Elderly, Blind, and Disabled (EBD) waiver.
Responsibilities
Medicaid & Case Management Coordination:
- Serve as the primary point of contact for Case Management Agencies (CMAs) to ensure seamless implementation and ongoing administration of Medicaid waiver programs.
- Coordinate care for clients under the Elderly, Blind, and Disabled (EBD) waiver and the In-Home Support Services (IHSS) program, ensuring strict adherence to program rules, service plan limits, and state regulations.
- Facilitate client intakes, reassessments, and care plan modifications by maintaining regular communication and reporting with case managers.
- Attend in-person or virtual service plan meetings with case managers, clients, and family members as required.
Coordination and Oversight:
- Coordinate and oversee the daily performance of homemakers, personal care workers (PCWs), and attendant care workers under IHSS to ensure adherence to Agency policies, procedures, and individualized care plans.
- Conduct client intakes, initial evaluations, and necessary supervisory follow-ups out in the field as needed.
- Document all administrative and coordination activities thoroughly—including dates, times, tasks, and outcomes—within our scheduling software.
- Provide re-training, targeted instruction, or administrative guidance based on field feedback to enhance care delivery.
- Address client and family concerns promptly and effectively in either English or Spanish, ensuring compliance with agency and Medicaid standards.
Training and Development:
- Assist in the initial orientation and training of new homemakers, PCWs, and IHSS attendant caregivers on Agency policies, compliance standards, and care plan requirements.
- Support ongoing training initiatives to elevate caregiver skills and remain compliant with state regulations for Medicaid waiver service delivery.
- Collaborate with leadership to update training materials reflective of shifting state regulatory requirements and best practices.
Compliance and Quality Assurance:
- Maintain compliance with state (CDPHE/HCPF) and federal regulations governing home care and Medicaid waiver programs, including EVV (Electronic Visit Verification) and documentation standards.
- Conduct periodic audits of client charts, task logs, and caregiver documentation within WellSky to verify accuracy, completeness, and regulatory compliance.
- Participate in quality assurance initiatives and audit preparation to optimize service delivery and client outcomes.
Communication and Collaboration:
- Maintain effective daily communication across internal departments (administration, nursing, and scheduling) to ensure seamless coverage and rapid issue resolution.
- Utilize WellSky (formerly ClearCare) daily to manage client records, coordinate schedules, track caregiver credentials, and monitor shift documentation.
- Provide bilingual (English/Spanish) translation and communication support between English-speaking management/case managers and Spanish-speaking clients, families, and caregivers.
- Act as a liaison between field caregivers, agency management, and external care team members to ensure comprehensive care support.
Qualifications:
- Minimum of 2 years experience in non-medical home care
- Bilingual fluency in English and Spanish (written and spoken) required.
- Ability to commute to the office daily.
- Reliable personal transportation, valid driver's license, and active auto insurance for required out-of-office tasks (such as client intakes and service plan meetings).
- Demonstrated experience or formal training in the home care industry or personal care services.
- Direct experience with Medicaid waiver programs, specifically the IHSS (In-Home Support Services) program and EBD (Elderly, Blind, Disabled) waiver required.
- Proven track record of working effectively with Case Management Agencies (CMAs) and case managers.
- Proficiency with WellSky (formerly ClearCare) software strongly preferred.
- Completion of required personal care services training.
- Thorough understanding of state regulations, EVV standards, and documentation guidelines applicable to non-medical home care.
- Exceptional communication, time management, and problem-solving skills.
Working Conditions:
- Primary work setting is in the agency office with daily on-site attendance.
- Local travel required for out-of-office duties, including client intakes and service plan meetings.

Pay: $25.00 - $27.00 per hour
