Overview:
The Licensed Practical Nurse (LPN) in Home Health provides basic nursing care to patients under the supervision of a Registered Nurse (RN). The LPN plays a critical role in monitoring patient health, administering medications, and supporting the overall patient care plan. This position requires strong organizational skills, clinical judgment, and a compassionate approach to care, as well as the ability to work independently in patients' homes.
Minimum Qualifications
- Graduate of an accredited LPN/LVN program.
- Current Licensed Practical Nurse (LPN) license in the state.
- Minimum of 1 year of clinical nursing experience, preferably in home health or a related field.
- Ability to follow care plans and clinical instructions accurately.
- Proficient in electronic health records (EHR) systems.
- Strong communication and interpersonal skills.
- Current CPR certification.
Job Responsibilities
- Assist in the development and implementation of individualized patient care plans.
- Provide basic nursing care, including wound care, injections, and administering medications as directed by an RN or physician.
- Monitor and document patient health, reporting changes to the supervising RN or physician.
- Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding, as necessary.
- Educate patients and families on basic care techniques, medications, and disease management.
- Support the Registered Nurse in coordinating with physicians and therapists to ensure comprehensive patient care.
- Maintain accurate and timely documentation in electronic health records.
- Comply with home health regulations and company policies.
- Ensure compliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.
- Perform any additional duties as assigned to meet the needs of the department and organization.
Physical Requirements
- Manual Dexterity: Ability to use medical equipment and perform nursing tasks for extended periods.
- Mobility: Ability to move between patients' homes, including bending, standing, and assisting patients.
- Lifting: Ability to lift or move objects and patients, typically up to 50 lbs.
