Job Summary
Make a meaningful difference every day—help patients heal, recover, and regain quality of life. We are seeking a skilled and compassionate Wound Care Nurse to lead wound management in our skilled nursing facility. This role is essential in delivering high-quality, patient-centered care through expert assessment, treatment, and prevention strategies. The ideal candidate will possess a strong background in wound care management and demonstrate exceptional clinical skills. The Wound Care Nurse will also be providing specialized care to patients with complex wounds, ensuring optimal healing and recovery through evidence-based practices. The hourly rate for this position starts at $40/hr. and increases based on experience. Join our team and make a difference from day one!
Assess and evaluate patients' wounds to develop individualized care plans
Provide direct care, including dressing changes and advanced wound therapies, using aseptic techniques to prevent infection
Monitor healing progress and adjust care plans as needed
Ensure compliance with infection control protocols and regulatory standards
Administer medications as prescribed and monitor patients for pain management
Educate patients and their families on wound care practices and prevention strategies
Collaborate with interdisciplinary teams to coordinate comprehensive patient care
Document all assessments, interventions, and patient responses accurately in medical records
Provide support in critical care transport situations as needed
Stay updated on the latest advancements in wound care treatment and techniques
Part of the nurse management on-call rotation
Active RN license in good standing
Wound care experience preferred (skilled nursing or long-term care strongly preferred)
Certification in wound care (e.g., CWCN or CWS) is a plus but not required
Strong clinical judgment and attention to detail
Excellent documentation and communication skills
Ability to work collaboratively in a team environment
