Overview
The Operations Care Coordinator serves as the central point of contact between clients, families, field caregivers, and agency leadership. This role combines hands-on care coordination with operational oversight ensuring services are delivered safely, documentation is compliant, and the agency continues to grow through community outreach and partnership development.
Duties
- Conduct client intakes, assessments, and in-home evaluations
- Develop, implement, and monitor individualized care plans
- Match caregiver qualifications and availability to client needs
- Coordinate caregiver introductions and client onboarding
- Serve as primary point of contact for clients and families
- Coordinate with physicians, case managers, MCOs, and healthcare providers
- Monitor client progress and adjust care plans as needed
- Handle after-hours and weekend emergency calls as needed
- Conduct home visits to assess service quality and client satisfaction
Qualifications
- Associate's degree or equivalent; 2+ years home care experience
- Knowledge of CHC, OLTL, ODP, Medicaid/MCO billing
- Strong organizational, communication & leadership skills
- Proficiency with EVV systems and electronic documentation
- Ability to manage multiple priorities under pressure
- Experience with scheduling and care planning platforms
- Professional presentation and community outreach skills
- Familiarity with PROMISe provider enrollment requirements
Pay: $20.00 - $28.00 per hour
