Your Office Is the Community
You'll provide comprehensive primary care throughout assisted living, memory care, skilled nursing, and senior housing communities across Willmar and surrounding Western Minnesota.
Every community is different.
Every patient is different.
And every day requires you to think clinically.
One visit may involve managing diabetes, CHF, COPD, or hypertension.
The next may require evaluating an acute change in condition, coordinating care after a hospitalization, reviewing medications, addressing a wound, or helping a family understand the next step in their loved one's care.
This is a role for a PA-C who enjoys solving clinical problems instead of simply moving through an appointment schedule.
What You'll Be Doing
- Conduct comprehensive patient assessments and routine provider visits.
- Diagnose and manage acute and chronic medical conditions.
- Develop individualized treatment plans based on patient needs and goals.
- Identify subtle changes in condition and intervene proactively.
- Manage chronic diseases and preventative care.
- Coordinate care following hospitalizations, rehabilitation stays, and specialist visits.
- Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and facility leadership.
- Educate patients and families regarding diagnoses, medications, treatment plans, and disease management.
- Communicate directly with senior living clinical teams regarding changes in patient condition.
- Participate in specialty programs and quality improvement initiatives.
- Maintain accurate and timely clinical documentation.
- Help reduce avoidable emergency department visits and hospitalizations through proactive medical management.

Required Qualifications
- Graduate of an accredited Physician Assistant program.
- Active Minnesota PA-C license in good standing.
- Strong assessment, diagnostic, and clinical decision-making skills.
- Ability to work autonomously while collaborating with physicians and interdisciplinary healthcare teams.
- Confidence managing patients with multiple chronic conditions.
- Strong communication and care coordination skills.
- Passion for serving older adults and medically complex populations.
- Reliable transportation and willingness to travel throughout Willmar and surrounding Western Minnesota communities.
