Caresify Home Health is seeking an energetic and results-driven Clinical Liaison / Business Development Representative to expand our referral network throughout the Philadelphia, Delaware County, and Lower Bucks County markets.
If you have established relationships with hospitals, physicians, skilled nursing facilities, rehabilitation centers, assisted living communities, and case managers, we’d love to speak with you.
Responsibilities
- Develop and maintain referral relationships with hospitals, physician practices, skilled nursing facilities, inpatient rehabilitation facilities, assisted living communities, dialysis centers, and case managers.
- Conduct daily face-to-face marketing visits to referral sources.
- Educate healthcare professionals about Caresify Home Health services.
- Identify qualified patients for Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, and Home Health Aide services.
- Coordinate with the intake team to ensure timely admissions.
- Meet and exceed monthly admission goals.
- Represent Caresify professionally at networking events and community outreach activities.
- Maintain accurate referral activity in the CRM.
Qualifications
- Minimum 2 years of home health marketing or healthcare business development experience preferred.
- Existing referral relationships within the Philadelphia region strongly preferred.
- Knowledge of Medicare home health eligibility and referral process.
- Excellent communication and relationship-building skills.
- Self-motivated with the ability to work independently.
- Reliable transportation and valid driver’s license.
