Steward Hospice Care, Inc. is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team as a Field Hospice LVN. In this role, you will provide direct patient care by conducting routine and PRN hospice visits, performing skilled nursing assessments, managing symptoms, and educating patients and families to ensure comfort and quality of life.

Key Responsibilities
- Provide routine and PRN skilled nursing visits in patients' homes, assisted living facilities, board and care homes, and skilled nursing facilities.
- Perform comprehensive nursing assessments and monitor changes in patient condition.
- Assess and manage pain and other symptoms according to the hospice plan of care.
- Administer medications and treatments as ordered by the hospice physician.
- Perform wound care, catheter care, medication management, and other skilled nursing procedures as appropriate.
- Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and end-of-life care.
- Collaborate with the interdisciplinary hospice team, including physicians, RNs, social workers, chaplains, hospice aides, and other healthcare professionals.
- Accurately document all patient assessments, interventions, and communications in the electronic medical record in accordance with Medicare Conditions of Participation and agency policies.
- Participate in on-call rotations as assigned.
- Report significant changes in patient condition promptly and communicate recommendations to the RN Case Manager and Hospice Medical Director.
Qualifications
- Bilingual in English and Korean required.
- Current and unrestricted California Licensed Vocational Nurse (LVN) license.
- Current BLS certification.
- Hospice, home health, SNF, or acute care experience preferred, but willing to train motivated candidates.
- Excellent assessment, communication, and organizational skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Reliable transportation, valid California driver's license, and current automobile insurance.
- Willingness to travel throughout the assigned service area.

Pay: $55.00 - $70.00 per hour
