We are excited to announce our Care Manager opening in Paramus, NJ! Integrity House seeks a Care Coordinator that will utilize skills and care coordination strategies to assist in aftercare placement of patients who will complete treatment episodes on the addiction’s treatment units, including the 54-bed Level 4.0 Medically Managed Intensive Inpatient and 40-bed Level 3.7 Medically Monitored Intensive Inpatient programs. This individual will leverage their experience in case management, care coordination and transitions of care to transform Care Plus Bergen’s care coordination model. They will be responsible for the care coordination and operational improvement activities across the continuum of care; working effectively with other clinical leaders and senior administration; and ensuring organizational and departmental success.
Primary Duties and Responsibilities of a Care Manager:
Key Responsibilities:
- Conduct initial screenings for substance use, mental health, and social determinants of health.
- Collaborate with physicians, nurses, and behavioral health specialists to develop individualized care plans.
- Review clinical documentation and assess patient readiness for various levels of treatment (inpatient detox, residential, skilled nursing facility, outpatient, etc.).
- Coordinate transitions of care between hospital units, treatment programs, and community resources.
- Serve as a primary point of contact for patients and families regarding treatment plans and discharge processes.
- Maintain regular communication with multidisciplinary teams (medical, psychiatric, and social work staff).
- Track patient progress and follow-up appointments to ensure continuity of care and reduce relapse risk.
- Facilitate safe discharge planning by linking patients to outpatient providers, recovery housing, peer support, and community programs.
- Support patient motivation for recovery through brief interventions, education, and resource navigation.
- Encourage engagement with harm reduction or medication-assisted treatment (MAT) when appropriate.
- Participate in daily rounds, multidisciplinary team meetings, and case conferences.
- Communicate effectively with external partners (community agencies, treatment facilities, probation/parole, etc.).
- Maintain accurate and timely records in the electronic health record (EHR)
- Ensure compliance with hospital policies, HIPAA, and federal regulations (e.g., 42 CFR Part 2 for substance use confidentiality).
- Track metrics related to patient outcomes, engagement, and readmissions.
- Maintain an updated directory of local treatment resources, peer recovery networks, and community services.
- Respond to patient crises or relapses within the hospital setting in collaboration with clinical staff.
- Coordinate with psychiatry, emergency services, or security as needed for patient safety and stabilization.
- Perform additional functions supportive of the program as assigned by the respective Program Director.
- Adhere to all safety and security protocol of the worksite.
Education/Experience:
- Bachelor’s Degree in Counseling or Social Work, required
- At least 2 years of experience in the Addictions and/or Mental Health field
- Proficient with Technology
- Must possess good written and verbal skills
Licensing or Certifications:
- Clinical Licensure preferred but not required (e.g., CADC/LCADC, LSW/LAC)
- A valid Driver’s License
Work Schedule:Tuesday through Saturday from 10am to 6pm
