Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Prepare nutritious meals and ensure proper nutrition according to care plans
- Monitor patient health by checking vital signs, observing symptoms, and reporting changes promptly
- Administer medications accurately following prescribed schedules and protocols
- Support patients with cognitive impairments such as dementia or Alzheimer’s through memory care techniques and patient observation
- Operate medical equipment safely, including Hoyer lifts and other assistive devices, ensuring proper use and safety
- Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with HIPAA regulations
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment for clients
- Collaborate with healthcare professionals to develop and implement personalized care plans
- Provide emotional support and companionship to foster a positive environment for clients.
- Transfers/Lifting, Diaper Change
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group homes preferred
- Knowledge of patient monitoring, medication administration, and vital signs measurement
- Certification in CPR and First Aid required; additional certifications, such as HIPAA training, are a plus
- Experience working with individuals with developmental disabilities, autism, dementia, or hospice needs is highly desirable
- Strong observational skills to monitor patient condition and recognize changes requiring intervention
- Excellent communication skills to effectively collaborate with clients’ families and healthcare teams
- Valid driver’s license preferred for transportation purposes; reliable transportation required
