Job Description
Collaborate with other nurse practitioners(s) from Colorado Primary Care Clinic, Inc. (CPCCI) to provide assessment and clinical management of patients. The Nurse Practitioner will provide recommendations and serve as a resource for the development of patient care standards, clinical decision-making, protocols, and procedures related to nursing practices for CPCCI. Job specifications related to the Nurse Practitioner's area of practice are outlined in the attached addendum as appropriate. A two year obligation and commitment to this position is required.
Educational Preparation
- Master of Nursing degree or higher as an FNP-BC
- Appropriate certification as a Nurse Practitioner from an accrediting body with (1) year post masters experience in clinical practice.
- Valid Certificate of Authority for Advanced Practice Nursing in the State of Colorado.
- Valid Certificate to Prescribe or eligibility to apply for provisional prescriptive privileges in the State of Colorado prior to hire.
- Current RN license to practice nursing in the State of Colorado.
- Maintain continuing educational requirements as determined by Colorado State Board of Nursing and credentialing body for advanced practice certification (i.e., American Nurse Credentialing Center or American Association of Nurse Practitioners)
Clinical Practice Obtain patient histories, perform physical examinations and identify normal and abnormal finding and develop an appropriate treatment plan.
- Evaluate and manage acute and chronic health problems and changes in status.
- Operationalize the patient's plan of care and perform patient follow up as needed.
- Perform procedures related to scope of practice and clinical specialty, if trained.
- Order and interpret diagnostic tests.
- Order specialty referrals when indicated.
- Order durable medical equipment (DME) when indicated.
- Order medications appropriate to patient condition with ongoing monitoring.
Colorado Primary Care Clinic, Inc. is a Patient Centered Medical Home (PCMH) via NCQA accreditation. All charting, quality metrics, patient activities, and quality assurance activities must adhere to NCQA standards of care.
- Provide consultation and education to all staff, nurse practitioner students, and family members (with patient permission) regarding patient care issues.
- Provide patient and family education.
- Utilize community resources and act as a liaison by relaying information necessary for continuity of care to other professionals and agencies.
Administrative Responsibilities Attend and participate in pertinent meetings associated with CPCCI.
- In addition, Prescriptive Authority Practices will be kept current by provider. per current Colorado State Board of Nursing requirements.
- Maintain professional liability insurance per Colorado State requirements.
- Maintain own DEA licensure
- Maintain own health insurance
Nurse Practitioner provides care to include but not limited to patient care as defined in the current Colorado Nurse Practice Act.
- Provide chronic care management for the patients. This includes office visits, group visits and phone management as condition requires.
- Collaborate with the patient's specialty physician, when appropriate clinical expert (endocrinologist, et al), the staff and all involved team members as needed for comprehensive patient care.
- Recommended to participate in interdisciplinary team meetings and assist with implementing interventions as needed.
Compensation is a percentage fee for service pay structure.

- You will be working as part of a nurse practitioner team.
- There is no benefit package associated with this position.
- There is no call obligation, no weekend obligation, no holiday obligation.
- The position is a full (preferred) or part time work week Monday to Friday during usual business hours with support staff.
- You will be required to complete a minimum of 130 encounters per month in a full time position, you will be required to complete 120 encounters per month in a part time position.
Projected Total Compensation: $99,999.00 - $100,000.00 per year
