About the Position
As a Primary Care Nurse Practitioner, you'll provide comprehensive healthcare services to an established panel of adult and geriatric patients in a modern outpatient clinic.
You'll focus on:
- Preventive medicine
- Wellness visits
- Annual Medicare Wellness Exams
- Chronic disease management
- Diabetes management
- Hypertension management
- Heart disease management
- COPD and asthma management
- Medication management
- Health screenings

This practice allows providers the time and support needed to deliver personalized, relationship-based care instead of rushing through high patient volumes.
Qualifications
We welcome experienced:
- Family Nurse Practitioners (FNP)
- Adult-Gerontology Nurse Practitioners (AGNP)
- Primary Care Nurse Practitioners
Requirements include:
- Active Arizona Nurse Practitioner License
- Board Certified Nurse Practitioner
- DEA License (or ability to obtain)
- Current BLS Certification
- ACLS Preferred
- Experience in Primary Care, Family Medicine, Internal Medicine, Adult Medicine, Geriatrics, Value-Based Care, Community Health, FQHC, or Ambulatory Care preferred
- Strong clinical assessment and diagnostic skills
- Excellent communication and patient education abilities
- Passion for caring for seniors and medically underserved populations
- Comfortable managing patients with multiple chronic conditions
- Ability to travel between nearby clinic locations as needed
