Duties
- Provide attentive patient observation to monitor health status and respond promptly to changes or concerns
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Support clients with Alzheimer’s care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities
- Operate Electronic Health Record (EHR) systems to document patient interactions, vital signs, and medical history securely in compliance with HIPAA regulations
- Perform medical tasks such as vital signs measurement, catheter care, Hoyer lift operation, and basic wound care as trained
- Prepare nutritious meals while adhering to dietary restrictions and safety standards
- Conduct cleaning and laundry on laundry days and as needed to maintain a safe and hygienic environment in the home
- Use medical terminology accurately when documenting patient care activities in medical records or reports
- Support hospice care needs by providing comfort measures and emotional support during end-of-life stages
Requirements
- Proven experience in home health caregiving or long-term care environments such as nursing homes or assisted living facilities
- Knowledge of medical terminology, medication administration, vital signs monitoring, and emergency procedures including CPR certification
- Experience working with individuals with disabilities or special needs; familiarity with Alzheimer's and dementia care is required
- Ability to operate EHR/EHR systems efficiently for accurate documentation of patient care activities
- Strong patient observation skills combined with compassionate caregiving abilities
- Valid certification in CPR/AED; additional certifications such as catheter care or hospice support are a plus
- Ability to work independently during weekend shifts while maintaining excellent communication with the healthcare team
- Commitment to maintaining patient privacy and adhering to HIPAA standards for confidentiality
