MDS Coordinator (RN or LVN) – Skilled Nursing Facility

Job Summary
We are seeking an experienced and detail-oriented MDS Coordinator (RN or LVN) to join our team. The ideal candidate will have prior MDS experience and a strong understanding of regulatory requirements, reimbursement systems, and interdisciplinary care planning within a skilled nursing or long-term care setting.
Responsibilities
- Complete accurate and timely MDS assessments (RAI process) in accordance with federal and state regulations
- Coordinate care plan meetings and collaborate with the interdisciplinary team
- Ensure compliance with Medicare/Medicaid guidelines and reimbursement requirements
- Monitor documentation to support case mix and quality outcomes
- Track and manage MDS schedules and submission deadlines
- Provide guidance and training to nursing staff on documentation practices
- Participate in audits, quality assurance, and performance improvement initiatives
Qualifications
- Active RN or LVN license in good standing (required)
- Previous MDS experience required
- Strong knowledge of RAI process, PDPM, and CMS regulations
- Skilled in assessment, care planning, and clinical documentation
- Excellent organizational and communication skills
- Experience in skilled nursing or long-term care preferred
