Position Overview
SweetSpot is seeking a Clinical Diabetes Specialist (CDCES) to play a key role in delivering high-quality remote diabetes care through our remote monitoring platform and clinical services. This role focuses on supporting patients and providers through ongoing glucose data interpretation, clinical insights, patient education, and collaborative care management.
The ideal candidate is passionate about improving diabetes outcomes, building meaningful provider relationships, and delivering compassionate patient care in a remote environment.
What You’ll Do
- Independently analyze and interpret CGM and/or insulin pump reports for patients with Type 1 and Type 2 diabetes through remote, asynchronous reviews.
- Provide timely clinical recommendations and documentation within partner EMR systems to support provider decision-making and continuity of care.
- Conduct follow-up outreach with patients as needed to review glucose trends, reinforce care plans, provide diabetes self-management education, and communicate provider-directed recommendations.
- Identify trends, barriers, and opportunities for improved glycemic outcomes and escalate clinical concerns appropriately.
- Support longitudinal patient engagement through ongoing remote monitoring, education, and proactive follow-up.
- Collaborate closely with providers to communicate patient progress, clinical outcomes, and opportunities for therapy optimization.
- Participate in multidisciplinary care coordination with internal clinical and patient engagement teams to support a seamless patient experience.
- Build strong collaborative relationships with clinic partners and providers to support clinical workflow integration and high-quality patient care.
- Share patient outcome trends, engagement insights, and program performance updates with providers and leadership teams.
- Partner with the Patient Engagement and Enrollment teams to support patient onboarding, retention, and continuity of care throughout the patient journey.
- Help identify workflow improvement opportunities that enhance both patient outcomes and operational efficiency.
Required Qualifications
- Licensed RN, RD, NP, or PA with diabetes management experience.
- Active, unrestricted CDCES certification.
- 5+ years of experience in diabetes care management.
- 5+ years of experience managing CGM and insulin pump therapy.
- Strong understanding of diabetes technology, medication management, and interpretation of glucose data.
- Excellent communication and patient education skills.
- Ability to work independently while collaborating effectively across remote teams.

- You have experience with Dexcom, FreeStyle Libre, Eversense, Medtronic, Tandem, and Omnipod, and iLet devices.
- You have experience navigating and documenting within EHR systems.
- You have experience in remote patient monitoring, digital health, or virtual diabetes care environments.
- You have the ability to manage multiple priorities in a fast-paced, evolving environment.
