Care United Home Health is seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our growing team. This role offers the opportunity to build meaningful relationships with patients while providing high-quality care in the comfort of their homes.

What You'll Do
- Conduct comprehensive patient assessments in the home setting.
- Develop and update individualized plans of care in collaboration with physicians and interdisciplinary team members.
- Coordinate patient care, including scheduling visits and obtaining physician orders.
- Educate patients and families on diagnoses, medications, treatment plans, and self-care techniques.
- Monitor patient progress and communicate significant changes to the healthcare team.
- Administer medications and monitor patient response to treatment.
- Maintain accurate and timely clinical documentation.
- Ensure compliance with agency policies, state regulations, and home health standards.
- Provide emotional support and guidance to patients and their families.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure optimal patient outcomes.
QualificationsRequired
- Active Indiana Registered Nurse (RN) license.
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
- Current CPR certification.
- Minimum of two (2) years of nursing experience.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong clinical assessment, communication, and organizational skills.
Preferred
- At least one (1) year of Home Health or Case Management experience.
- Experience with electronic health record (EHR) systems.
- OASIS documentation experience.

Working Conditions
- Travel throughout the assigned service area to provide patient care in the home.
- Combination of field-based and occasional office responsibilities.
- Ability to perform physical nursing duties, including standing, walking, bending, and assisting patients as needed.
- Occasional on-call or weekend coverage may be required based on patient needs.
