Home Health LPN
We’re currently seeking a Home Health LPN to join our amazing team.
Details of the Opportunity:
- Anticipated start date: ASAP
- Salary range: $47/hour
- Shift: Days
- Assignment Length: 13 weeks

Daily Range of Responsibilities:
- Perform in-home patient assessments and OASIS documentation
- Administer medications, treatments, and wound care
- Educate patients/families on home care and safety
- Coordinate care with interdisciplinary team
- Prioritize independent clinical decision-making in patient homes
- Utilize Home Base Home Care software for documentation and care coordination
Job Qualifications:
- Graduate of an accredited Nursing program
- Active LPN license in the state of practice (or compact license)
- Current BLS certification
- Specialty Certifications (preferred): Wound Care, IV Therapy, OASIS Documentation, Home Base Home Care Charting
- Experience: 1+ year home health experience
