Responsibilities may include:
- Performing duties as outlined in the plan of care such as Medication Administration, Personal Care, Light Housekeeping and day to day support.
- 2:2 Ratio of individuals through a consistent caseload.
- Effectively communicating updates and changes to patient's status to the supervising RN.
- Maintaining a safe patient environment.
- Providing emotional and social support and compassionate care to patients.
- Maintaining current patient records and progress notes.
- Working as a member of the interdisciplinary team.
- Developing and establishing a plan of care with physician.
Requirements for this position include:
- Minimum of an associate's degree or equivalent in Nursing from an accredited nursing program.
- Nursing experience preferably within Private Duty Nursing, Home Health or Disability Services specialty is a plus but not required
- Valid driver's license, car insurance and reliable transportation.
