Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient condition through observation of vital signs and overall health status
- Administer medications accurately following medical instructions and maintain detailed medical documentation
- Support clients with medical procedures such as catheter care, wound care, and managing medical equipment like Hoyer lifts
- Maintain patient confidentiality by adhering to HIPAA regulations and safeguarding sensitive information
- Document patient progress using Electronic Health Record (EHR) systems and ensure compliance with home healthcare regulations
- Provide companionship and emotional support, especially for clients with dementia or experiencing end-of-life care needs
- Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment
- Support patient transfer methods safely using proper techniques and equipment such as patient lifts or transfer belts
- Respond promptly to emergencies by applying first aid or basic life support (CPR) when necessary
Skills
- Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or memory care
- Strong knowledge of medical terminology, medication administration, and vital signs monitoring
- Skilled in patient observation, communication, and providing personalized care tailored to each client’s needs
- Familiarity with EMR/EHR systems for accurate documentation of patient records and medical history
- Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts or mobility aids
- Excellent understanding of home health care regulations and compliance standards including HIPAA guidelines
- Experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly desirable
- Capable of maintaining a compassionate attitude while managing multiple responsibilities in fast-paced environments
- Knowledge of dementia care techniques, hospice practices, and end-of-life support strategies
