Meadowbrook at Appleton is looking for a compassionate and dedicated Social Services team member to help provide emotional, social, and advocacy support to our residents and their families.
This role plays an important part in ensuring our residents feel supported, heard, and cared for while promoting their overall quality of life.
In this role, you will:
- Support residents and families through admission, adjustment, care planning, and discharge planning
- Complete social histories and psychosocial assessments
- Advocate for resident rights, dignity, and emotional well-being
- Work closely with the interdisciplinary team to develop individualized care plans
- Assist residents and families with education, resources, problem-solving, and community referrals
- Participate in quality assurance and policy review meetings
- Ensure accurate documentation related to residents’ psychosocial needs
- Provide support during difficult transitions, placement concerns, and significant life events
- Educate staff on resident rights, abuse prevention, cultural diversity, aging, disability, and the role of Social Services
