Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in various clinical settings, including long-term care, assisted living, hospital environments, and specialized areas such as ICU, emergency medicine, and geriatrics. The LPN will play a vital role in providing high-quality patient care, performing assessments, administering medications, and supporting treatment plans across diverse patient populations. Responsibilities
- Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations.
- Administer medications and injections accurately following physician orders and safety protocols.
- Assist with invasive procedures such as IV insertion, catheterization, tube feeding, and sterile dressing changes.
- Monitor patients’ conditions closely, including ventilator management, airway management, and acute pain management.
- Document all patient interactions accurately using EMR/EHR systems like Epic, Cerner, or Athenahealth while maintaining HIPAA compliance.
- Support discharge planning and coordinate care with multidisciplinary teams for post-acute and outpatient services.
- Provide specialized care for patients with dementia, Alzheimer’s disease, memory care needs, or behavioral health issues.
- Manage infection control practices and aseptic techniques to ensure patient safety.
- Educate patients and families on health coaching, medication administration, nutrition, and disease management strategies.
- Collaborate with physicians in specialty areas such as cardiology (cardiac catheterization), ophthalmology, neonatology (neonatal ventilator), or occupational health as needed.
- Participate in case management activities including discharge planning and follow-up care coordination for diverse populations including pediatrics and geriatrics.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Proven experience working with diverse patient populations such as long-term care residents, ICU patients (Level I & II Trauma Centers), emergency room cases, or post-operative patients.
- Knowledge of ICD-9/10 coding systems and medical terminology relevant to various specialties including hospice care, infectious disease care, and medical-surgical units.
- Proficiency in EMR/EHR systems such as Epic, Cerner, eClinicalWorks or Athenahealth; experience with medical documentation is highly preferred.
- Strong understanding of physiology, anatomy, pharmacology (including medication administration protocols), and diagnostic evaluation techniques.
- Ability to perform venipuncture, phlebotomy, suturing, sterile processing techniques, and specimen collection/processing efficiently.
- Excellent patient assessment skills combined with compassionate communication tailored to individuals with disabilities or developmental challenges.
- Critical thinking skills for triage situations and acute care scenarios including ICU or emergency medicine settings.
- Ability to work independently or as part of a multidisciplinary team in fast-paced environments such as hospitals or outpatient clinics.
