Working 40 hours per week
Working Monday through Friday
Rates: EXTREMELY competitive rates/salary
Responsibilities:
- Assessment of patient and conditions as per homecare guidelines
- Collaborate with physicians and nurses to devise individualized care plans for patients
- Perform Start of Care visits as need for patients
