Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary
- Monitor and document vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately
- Provide medication reminders and assist with medication administration following medical instructions and HIPAA regulations
- Observe patient conditions closely, noting any changes or concerns related to dementia, Alzheimer's disease, or other cognitive impairments
- Support patients during medical procedures such as catheter care or hospice-related activities while maintaining dignity and comfort
- Maintain detailed medical records using Electronic Health Record (EHR) systems and ensure all documentation complies with privacy standards
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment in accordance with home & community care standards
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated ability to work with individuals with developmental disabilities or memory care needs such as dementia or Alzheimer's disease
- Knowledge of medical terminology and familiarity with medical documentation practices is essential
- Certification in CPR and first aid is required; additional certifications like hospice care or catheter care are advantageous
- Experience operating EMR (Electronic Medical Records) or EHR systems for accurate record keeping
- Proven ability to handle long-term care responsibilities, including patient observation, medication administration, and working with diverse populations such as seniors or individuals with disabilities
- Strong communication skills to collaborate effectively with healthcare teams while maintaining patient confidentiality under HIPAA guidelines
