We are looking for a Nutritionist/Nutrition Consultant to collaborate with our Sevita family across our Adult IDD population, including SLS homes, FHA homes, Group Home settings, and Day programs, as an intricate component of our success.
The Nutritionist will assess nutritional needs, develop individualized nutrition recommendations, provide nutrition education, support healthy meal planning, and collaborate with staff and care teams to promote the overall health and well-being of the individuals we serve.
Essential Functions and Responsibilities:
- Conduct nutritional assessments and identify individual dietary and nutritional needs.
- Develop individualized nutrition plans and dietary recommendations based on each person’s needs, preferences, and goals.
- Provide nutrition education and consultation to individuals, families, caregivers, and staff.
- Assist providers with implementation of nutrition and dietary recommendations through consultation and coaching.
- Review menus and provide recommendations to promote balanced, nutritious, and appropriate meals.
- Provide guidance regarding portion sizes, hydration, food choices, food safety, and healthy eating habits.
- Monitor nutritional status, weight changes, appetite, hydration, and other nutrition-related concerns.
- Identify and provide recommendations regarding special diets, food allergies, dietary restrictions, and other nutritional needs.
- Collaborate with interdisciplinary teams, healthcare providers, caregivers, and other professionals.
- Participate in provider wraparound and consultation meetings by reporting nutritional assessment results and providing recommendations within areas of expertise.
- Provide training to IDD/BH providers and community members on nutrition, healthy eating, meal preparation, and dietary support.
- Support providers with implementation and monitoring of individualized nutrition plans.
- Review and evaluate documentation related to nutritional services and dietary recommendations.
- Promote person-centered care by incorporating individual preferences, cultural considerations, and personal goals into nutrition planning.
- Participate in reviewing and debriefing critical/safety incidents when nutritional concerns are identified.
- Complete all required documentation in a timely and accurate manner.
- Report any suspected abuse, neglect, or exploitation to the appropriate supervisor or department director and to appropriate state authorities as required by state rules and regulations.
- Comply with all organization standards to assure the health, safety, dignity, and confidentiality of all individuals.
Knowledge, Skills and Abilities:
- Knowledge of nutrition, dietetics, meal planning, and healthy eating practices.
- Knowledge of special diets, food allergies, dietary restrictions, and nutritional interventions.
- Knowledge of person-centered care and support for individuals with intellectual/developmental disabilities.
- Knowledge of health and wellness practices related to nutrition.
- Knowledge of food safety and sanitation standards.
- Knowledge of California Division of Developmental Services (DDS) and Department of Health Care Services (DHCS) preferred.
- Knowledge of Regional Center policies, procedures, and operations preferred.
- Ability to communicate nutritional information effectively to individuals, families, caregivers, and staff.
- Strong assessment, documentation, organizational, and communication skills.
- Ability to work collaboratively with interdisciplinary teams.
- Ability to develop practical and individualized nutritional recommendations.
Qualifications:Minimum:
- Bachelor’s degree in Nutrition, Dietetics, Food and Nutrition, or a related field; or equivalent education and experience, as applicable.
- Experience providing nutrition education, dietary consultation, or nutritional support.
- Experience working with individuals with intellectual and developmental disabilities preferred.
- Knowledge of nutrition assessment and individualized meal planning.
- Knowledge of special diets and nutritional accommodations.
- Valid CPR and First Aid Certification.
- Valid Driver’s License and auto insurance, if required for the position.
- Professional Liability Insurance, if required.
Preferred:
- Registered Dietitian Nutritionist (RDN) or Licensed/Registered Dietitian credential, where applicable.
- Experience working in IDD, behavioral health, healthcare, residential, or community-based settings.
- Experience developing individualized nutrition plans.
- Experience providing staff training and consultation.
- Knowledge of California DDS and Regional Center systems.
Pay: From $150.00 per hour
Expected hours: No less than 40.0 per week
