South Mountain Post Acute, a Medicare 5-Star Rated Skilled Nursing Facility, is seeking Arizona-licensed Registered Nurses to join our rehab and sub-acute units on the NOC shift.

What you’ll do:
- Provide comprehensive nursing care, including assessments, IVs, and catheterization
- Administer medications and prescribed treatments (including tube feedings)
- Monitor patient conditions and respond to changes appropriately
- Collaborate with interdisciplinary teams in rehab and sub-acute settings
- Document care accurately using electronic health records
- Educate patients and families on care plans and health management
- Experience with vents and trachs is a strong plus
Qualifications:
- Active Arizona RN license
- Skilled nursing or sub-acute experience preferred
- BLS required
