Key Responsibilities
As a Clinical Care Coordinator, you’ll play a central role in supporting our clinical team and ensuring smooth, efficient care delivery:
- Communicate promptly and professionally with clinicians, physicians, discharge planners, patients, and caregivers
- Create and update authorizations and orders for new and existing members with accuracy and timeliness
- Research and resolve authorization and order processing issues, discrepancies, and denials
- Notify physician offices and clinical staff of required documentation; investigate and follow up on missing paperwork or denied requests
- Receive, process, and follow up on admitting and discharge paperwork, including consent forms
- Assign and schedule clinical and therapy evaluations and visits
- Assist the Director of Nursing in tracking field clinician schedules, patient re-hospitalizations, and discharges from service
- Maintain strict confidentiality of member information in accordance with company policies
- Complete organizational office tasks including appropriate documentation filing
- Organize receipt of incoming faxes and upload documents appropriately
- Contribute to quality improvement initiatives and perform other duties as assigned, consistent with skills and training
Qualifications
- Experience with electronic medical record (EMR) systems
- Strong organizational and communication skills
- Ability to manage multiple tasks and resolve operational issues independently
- Prior experience in home health, healthcare coordination, or administrative support preferred
