Conduct comprehensive in-home patient assessments

Develop and update individualized care plans

Manage medications, wound care, and chronic disease support

Educate patients and families on self-care and health management
-
Coordinate care with physicians and interdisciplinary teams

Supervise and support LPNs and Home Health Aides

Ensure accurate, timely documentation of patient progress

Monitor and adjust care based on patient condition changes

Active Registered Nurse (RN) license
-
Prefer a minimum of 2 years of nursing experience (Home Health preferred)

Strong clinical judgment and assessment skills

Ability to work independently in the field

Excellent communication and patient education skills

Valid driver’s license, reliable transportation, and auto insurance

Current CPR certification

Experience with HomeCare HomeBase preferred (not required)
