PT FULL TIME
Upland Rehabilitation & Care Center is seeking a FULL TIME PT house therapy team. We are a super busy 206 bed skilled nursing facility located in the heart of the Inland Empire. We have a busy sub-acute wing as well as outpatient programs, and our mission is to change and improve the way skilled therapy is delivered. Our beautiful facility has a spacious gym with great equipment and ADL spaces. Experience/Qualifications:
Licensed Physical Therapy
This position is open to PT's at all experience levels
Some of the job duties for this position include:
- Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
- Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
- Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.Pay is negotiable.
- Performs other duties as may be assigned.
