Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record patients’ vital signs and overall health status regularly
- Prepare nutritious meals tailored to individual dietary needs and preferences
- Administer medications accurately following care plans and HIPAA regulations
- Support clients with mobility using equipment such as Hoyer lifts or transfer belts, ensuring safety during movement
- Conduct patient observation to identify changes in condition or behavior, reporting findings promptly
- Maintain a clean, safe, and comfortable environment through regular cleaning and organization tasks
- Follow detailed care plans designed by healthcare professionals to meet each client’s unique needs
- Provide emotional support and companionship to promote mental well-being
- Collaborate with healthcare teams, social workers, and family members to ensure comprehensive care delivery
- Adhere strictly to all safety protocols including CPR, First Aid, and HIPAA compliance
Qualifications
- Prior experience in home health, assisted living, nursing homes, or group home settings is highly preferred
- Demonstrated knowledge of patient care techniques including assisting with ADLs and medication administration
- Certifications such as CPR, First Aid, or hospice care are advantageous
- Experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer’s), or memory care needs is desirable
- Ability to lift heavy objects safely and perform physical tasks required for patient transfers using equipment like Hoyer lifts or resident’s <PERSON> transfers
- Strong observational skills for patient monitoring and reporting changes in condition accurately
- Excellent communication skills for working effectively with clients, families, and healthcare teams
- Valid driver’s license preferred for travel between client locations; must comply with all safety standards including HIPAA regulations
