JOB DESCRIPTION
POSITION: Nurse Practitioner
SUPERVISOR: Medical Director

- JOB SUMMARY: Provides health care to health centers’ patients according to the policies/procedures of CAA and within the scope of practitioner’s licensure and experience
- SPECIFIC DUTIES:
ESSENTIAL FUNCTIONS
- Provide primary care in accordance with established health center protocols within the practitioner’s scope of practice and current collaboration agreement and the Ohio scope of practice guidelines
- Routinely screens for cognitive impairment, common mental health problems and behaviors that compromise health, using evidence-based and developmentally appropriate tools
- Contributes to quality improvement activities and progress toward goals as assigned by the Medical Director and health center’s supervisory staff
- Complete and submit in a timely manner all patient charges
- Serve as clinical preceptor as assigned by the Medical Director or COO
- Applies relevant practice guidelines in provision of care
- Participate in community outreach programs as assigned by Medical Director or COO
- Provides services as required by grants and other funding sources
- Travels as required for trainings, continuing educationopportunities, to satellite sites, etc.
- Follows CAA policies/procedures, including current HIPAA regulations
- Collaborates with patients, families, the care team and other professionals to make decisions about treatment and attaining health goals
- Prepares for, participates in and evaluates effectiveness or huddles and other team meetings
- Meets formally and informally with the Medical Director and other providers to further the principles of patient centered care
- Considers both clinical and cost/utilization aspects in shared decision making
- Supports patient centered care by electronically organizing and documenting problem list, care plan, patient goals, and team communication
- Trains or assigns team member to coordinate care of individual patients and support family members
- Works well with others, demonstrating flexibility, practicality, and adaptability while emphasizing treatment goal achievement versus rigid adherence to treatment models
- Maintains compliance with all laws, regulations, and policies as defined by Federal, State and any other regulatory body
- Other duties as assigned
C. REQUIRED JOB QUALIFICATIONS:
1. Graduate of accredited Nurse Practitioner program
2. Current RN license from Ohio State Board of Nursing
3. Current certification from national certifying organization approved by the Ohio Board of Nursing
4. Current certificate of authority by Ohio Board of Nursing
5. Participation in committees, collaboratives, health fairs and other assigned programs
6. Maintains favorable status with collaborating providers
7. Prescribes medications as permitted by current Ohio regulation
8. Follows all OSHA and universal precaution regulations
9. DEA license
10. Annually verify the licensure and, if applicable, certification status of each collaborating physician with whom the nurse has an effective standard care arrangement
11. Annually or more often reviews each effective standard care arrangement jointly with at least one collaborating physician
ADDITIONAL REQUIREMENTS AND CONDITIONS OF EMPLOYMENT:
- At least three years nursing experience in ambulatory care setting preferred
- Effective interpersonal communication skills; ability to listen actively and effectively; uses patient’s preferred means of communication
- Current ACLS certification
- Background check including Ohio Bureau of Criminal Investigation, Medicare exclusion list, verification of valid driver’s license, employment medical exam, including a TB test, adherence to all CAA policies/procedures and requirements of all funding sources. CAA is a drug/alcohol-free and smoke-free workplace and post-offer drug and alcohol testing is required.

PHYSICAL REQUIREMENTS:
- Ability to lift/carry up to 50 pounds
- Ability to push, pull, carry, lift, bend, twist, reach, sit, stand for lengthy periods of time; ability to walk for extended periods of time
- Ability to work in a fast-paced and often high-stress environment as well as the ability to adapt quickly to workload changes
- Ability to deal with inside and outside clientele in a diplomatic and courteous manner while assuring adherence to CAA policies/procedures

- Ability to work with potentially hazardous medical substances and equipment
- Frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations, radiation and other conditions common to a clinical environment

- Above-average oral, reading and writing skills to accurately operate a computer, perform necessary documentation and effectively communicate with staff, patients, clients, customers, business partners, other CAA departments and visitors
- Ability to remain calm and cheerful under stressful working conditions
Job Types: Full-time, Contract
Pay: From $105,000.00 per year
