You'll provide skilled in-home therapy services under the direction and supervision of a licensed Physical Therapist, helping patients regain strength, mobility, and independence while delivering exceptional care wherever they call home.

- Schedule: M-F 8am-5pm
- Salary: From $52,520.00 per year
- Sign on Bonus: $5,000

You will...
- Provide skilled physical therapy services in patients' homes under the direction of a Physical Therapist
- Follow and implement individualized treatment plans
- Monitor, document, and report patient progress and changes
- Educate patients and caregivers on therapy techniques, safety, and equipment use
- Collaborate with physicians, therapists, and interdisciplinary care teams
- Complete accurate, timely documentation in compliance with Medicare, federal, and agency guidelines
- Participate in case conferences, training, and quality improvement activities

- Licensure: Current, unrestricted Physical Therapist Assistant (PTA) license in the state of employment.
- Experience: Minimum of 1 year of experience as a PTA in an acute care setting is required. Prior home health or community-based care experience is preferred.
- Skills: Demonstrated proficiency in physical therapy best practices, independent clinical judgment, acute problem-solving, and time management.
- Other Requirements: Valid driver's license, reliable transportation, and current auto insurance.
