B&D Integrated Health Services East is seeking an experienced and compassionate Tailored Care Management (TCM) Supervisor to lead and support our growing team of care managers. This hybrid role requires 2 days per week in-office and offers the opportunity to make a meaningful impact in the lives of individuals with Mental Health (MH), Substance Use Disorders (SUD), Intellectual/Developmental Disabilities (I/DD), and Traumatic Brain Injuries (TBI).
Key Responsibilities
- Supervise, coach, and support a team of care managers.
- Ensure compliance with Tailored Plan, LME/MCO, Medicaid, and state requirements.
- Review caseloads, participate in case reviews, and address escalated member care concerns.
- Monitor quality, outcomes, and documentation standards.
- Support staff training and professional development.
- Collaborate with community partners, Tailored Plans, and LME/MCOs.
Qualifications
One of the following is required:
- Bachelor's degree in Human Services or related field plus 5 years of care management/case management experience with I/DD or TBI populations; OR
- Master's degree in Human Services field plus 3 years of relevant experience.
Preferred Qualifications
- Supervisory experience in care management or case management.
- Experience with NC Medicaid 1915(i) services (required).
- Tailored Care Management experience preferred.
- LCSW/A, LCMHC/A, LCAS/A, or RN preferred.
Required Experience
- 2+ years of Social Work experience.
- 1+ year of I/DD services experience.
- Knowledge of behavioral health, physical health, I/DD, TBI, and North Carolina Medicaid services.
