Hamilton Place Personal Living, LLC is a 55 bed personal care home offering memory care and personal care services. We are looking for a Wellness Director to join our team!

Duties include but are not limited to:
- Plans and manages patient care according to each patient’s needs.
- Makes CNA/CMA monthly schedule
- Interviews patients and records their medical history and physical condition.
- Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration.
- Provides routine care for patients.
- Administers medications
LPN license and 1-2 years recent nursing experience in Personal Home/Assisted Living community required.
