Responsibilities
- Assist clients with activities of daily living (ADLs), including personal care, grooming, bathing, dressing, and mobility support.
- Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately in compliance with HIPAA regulations.
- Support medication administration and ensure proper documentation according to care plans and medication protocols.
- Facilitate meal preparation and assist with eating to meet nutritional needs while respecting dietary restrictions.
- Operate patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe movement and prevent injury.
- Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met.
- Provide transportation assistance for appointments, outings, or community activities to promote social engagement and independence.
- Follow individualized care plans tailored to each client’s needs, including dementia care, Alzheimer’s care, memory care, hospice care, or autism support.
- Engage in patient observation and communication to monitor behavioral changes or health concerns proactively.
- Collaborate with healthcare professionals, social workers, families, and team members to deliver comprehensive support aligned with each client’s goals.
Skills
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Strong knowledge of patient monitoring techniques including vital signs measurement and dementia or Alzheimer’s care strategies.
- Familiarity with HIPAA regulations ensuring confidentiality of client information at all times.
- Ability to perform patient transfer methods safely using Hoyer lifts or other assistive devices; manual handling skills are essential.
- Experience supporting individuals with neurodevelopmental disorders such as autism or developmental disabilities is advantageous.
- Excellent communication skills for effective client interaction and team collaboration.
- Certification in CPR and first aid is required; additional training in hospice care or geriatrics is a plus.
- Capable of heavy lifting when necessary while maintaining proper body mechanics for safety.
- Knowledge of medication administration procedures following prescribed protocols.
- Demonstrated patience, empathy, and a positive attitude when working with diverse populations across various age groups.
