Nurse Practitioner/Physician Assistant
Work directly with patients to diagnose and manage most common and many chronic illnesses;
- Functions independently to perform age appropriate history and physical examinations;
- Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs;
- Provide counseling and education;
- Prescribes appropriate pharmacologic and non-pharmacologic treatments;
- Collaborate with the physician to create, manage and maintain an integrated member care management plan of care, including self-management activities;
- Educate patients about preventative care;
- Facilitates staff, patient and family decision making by providing educational tools;
- Collaborates with multidisciplinary team members by making appropriate referrals;
- Comply with HIPAA rules and regulations;
- Participate in staff meetings, educational events, on-site and off-site training;
Minimum required credentials for Nurse Practitioner or Physician Assistant:
- NYS NP or PA degree required
- Graduate of an accredited Program
- Current certification in Basic Life Support (BLS).
- Advanced Cardiac Life Support (ACLS); and Pediatric Advanced Life Support (PALS) preferred
- Excellent oral and written communication skills
- Strong understanding of electronic health record a plus(eClinicalWorks)
- Competence in the use of computers, email, texting, and the current versions of Microsoft Office and other commonly-used business software
- One years’ experience preferred, not required
- Ability to work independently
- Word, Excel, Outlook skills required
Salary commensurate on experience. Position is located in the Setauket, Stony Brook area.
